We're seeking a skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health RN, you'll provide admission, case management, and follow-up skilled nursing visits for our patients. Your expertise will help ensure our patients receive the highest quality care in the comfort of their own homes.

Key Responsibilities:

  • Administer ongoing care and coordinate appropriate care for patients
  • Report patient care/condition/progress to the patient's physician and Clinical Manager on a continuous basis
  • Provide admission, case management, and follow-up skilled nursing visits for home health patients

Requirements:

  • Diploma, Associate, or Bachelor Degree in Nursing
  • Minimum of one year nursing experience
  • Strong organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation
