Job Summary

We are seeking a skilled Clinical Care Coordinator to join our team at UnitedHealthcare. As a key member of our healthcare team, you will play a vital role in ensuring our members receive the highest quality care and support.

Key Responsibilities
  • Discharge Planning: Collaborate with healthcare providers to develop and implement effective discharge plans, ensuring a smooth transition for our members.
  • Case Management: Utilize company and community-based resources to establish a safe and effective discharge case management plan for members.
  • Communication: Communicate with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
  • Patient Advocacy: Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
  • Documentation: Document all member assessments, care plans, and referrals provided.
Requirements
  • Active LVN License: Must have an active LVN license in the State of Texas or a non-4 year degree and 5+ years of experience working with ABD/SSI population within a clinical healthcare role.
  • Clinical Experience: 3+ years of clinical experience in a healthcare setting, preferably inpatient.
  • Technical Skills: Familiarity with Microsoft Office, including Word, Excel, and Outlook.
  • Reliable Transportation: Reliable transportation and the ability to travel in the assigned region to visit Medicaid members in various settings.
