Clinical Care Coordinator

We are seeking a skilled Clinical Care Coordinator to join our team at UnitedHealthcare. As a key member of our healthcare team, you will play a vital role in ensuring our members receive the highest quality care and support.
Key Responsibilities- Discharge Planning: Collaborate with healthcare providers to ensure a smooth transition of care for our members, ensuring all necessary services are arranged prior to discharge.
- Case Management: Utilize company and community-based resources to develop a comprehensive care plan for our members, addressing their unique needs and goals.
- Communication: Effectively communicate with members, families, and healthcare providers to ensure seamless care coordination and address any concerns or barriers.
- Advocacy: Advocate for our members' needs and preferences, ensuring their voices are heard and their care is tailored to their individual needs.
- Documentation: Maintain accurate and up-to-date records of member assessments, care plans, and referrals.
- Active LVN License: Possess an active LVN license in the State of Texas or equivalent experience.
- Clinical Experience: 3+ years of clinical experience in a healthcare setting, preferably inpatient.
- Technical Skills: Proficient in Microsoft Office, including Word, Excel, and Outlook.
- Travel Requirements: Reliable transportation and ability to travel locally to visit members in various settings.
