Job Description

Baptist Health System - San Antonio TX is seeking a skilled Clinical Care Coordinator to provide direct clinical patient care in Harlingen, Texas.

This role provides high-quality, safe, cost-effective, total nursing care to all patients. As a Clinical Care Coordinator, you will assume responsibility for assessing, planning, implementing, and evaluating the effectiveness of direct clinical care to assigned patients on a per shift basis.

You will work closely with the healthcare team to ensure seamless coordination and integration of patient care, based on established standards of safety, risk management, quality improvement, and infection control. Your goal will be to discharge patients with an optimum level of care provided.

The successful candidate will administer medications safely and in a timely manner, adhere to hospital policies, and perform HICS responsibilities as directed. You will also be responsible for knowing hospital emergency codes and adhering to hospital policies and procedures for each code.

Compensation
