At HighPointe Assisted Living & Memory Care, we are seeking a skilled Wellness Nurse LPN to join our team.

**Job Summary:**

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence.

**Responsibilities:**

* Assist in coordination of the resident admission process, including screening, assessment, and care needs
* Ensure care is delivered as indicated in resident care plans
* Medication and treatment administration, ensuring medications are administered per physicians' orders
* Assistance with coordinating care and services with other outside providers, including resident physicians
* Document and report care provided and resident observations, including unusual or significant changes
* Maintain a working knowledge of state regulations, policies, and procedures

**Requirements:**

* LPN licensure
* Strong clinical skills and knowledge of state regulations
* Excellent communication and teamwork skills
* Ability to work effectively in a fast-paced environment
