Job Description

At HighPointe Assisted Living & Memory Care, we strive to provide a supportive environment for our residents to thrive. As a Wellness Nurse LPN, you will play a vital role in ensuring the health and well-being of our residents.

Key Responsibilities:

  • Assist in the coordination of resident admission processes, including screening, assessment, and care needs.
  • Ensure that care is delivered as indicated in resident care plans.
  • Administer medications and treatments as prescribed by physicians.
  • Coordinate care and services with outside providers, including resident physicians.
  • Document and report care provided and resident observations.
  • Maintain a working knowledge of state regulations, policies, and procedures.

Requirements:

  • Graduate from an accredited school of practical nursing.
  • Current nursing license and state registration (LPN or RN).
  • Three years' experience in assessment and geriatric nursing, preferred.
  • A big smile, eagerness to learn, and compassion for our residents.
