About the Role:

  • This full-time position involves working closely with our interdisciplinary team to provide palliative care and hospice services to patients and their families in Texas.
  • You will be responsible for conducting face-to-face assessments, consulting with our Medical Director, and collaborating with our IDT members to develop and implement personalized care plans.
  • Your expertise in pain and symptom management, psychology of loss, and understanding of hospice principles will be invaluable in providing high-quality patient care.

Requirements:

  • Current licensure as an Advanced Practice Nurse in the state of Texas
  • A minimum of 3 years of nursing experience
  • Comprehensive knowledge of medicine, pharmacology, pain and symptom control, and psychology of loss
  • Valid Texas Drivers License and proof of current Automobile Liability Insurance
  • Excellent communication and interpersonal skills
