Role Summary:

The Hospice RN/RN Case Manager will serve as a key member of our interdisciplinary team, providing comprehensive care to patients with life-limiting illnesses. As a RN, you will be responsible for assessing patient needs, developing individualized care plans, and coordinating services with healthcare providers and support staff.

Key Responsibilities:

  • Conduct thorough patient assessments to identify medical, emotional, and spiritual needs
  • Develop and implement individualized care plans that address patient goals and priorities
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless delivery of care
  • Provide education and support to patients and families on end-of-life care, disease management, and symptom control
  • Maintain accurate and detailed clinical records, including progress notes and treatment plans

Requirements:

To be successful in this role, you should possess:

  • A valid RN license in the state of employment (or eligibility to obtain)
  • 1+ years of experience in hospice, home health, ICU, geriatric, or oncology nursing
  • Excellent communication and interpersonal skills
  • A commitment to delivering high-quality, patient-centered care
