Position Overview:

The Care Navigator is responsible for efficiently managing inquiry and referral communications. This role acts as a crucial link between referral sources, medical professionals, and hospital staff involved in transfer requests.

Key tasks include entering call information into the facility's database and managing electronic communications to ensure timely processing of referrals.

This position demands effective communication with healthcare providers and admissions teams, adhering to established hospital protocols for referral management. Flexibility in scheduling is essential, with opportunities for a hybrid work model.

Work Schedule:

Sunday to Thursday, 10:00 AM - 6:30 PM

Essential Duties:

1. Manage all incoming referral and inquiry calls, providing timely patient assessments.
2. Effectively engage with at-risk individuals and families through various communication channels.
3. Maintain accurate records in the facility database and utilize electronic systems for bed management.
4. Ensure that all patient medical records are complete and that clinical staff are informed of patient histories.
5. Provide accurate information to referral sources as needed.
6. Collaborate with patients, families, and community resources, as well as internal teams.
7. Adapt to departmental needs, which may include varying shift hours.

Qualifications:

Education:

Required: RN License or Master's Degree

Experience:

1-3 years in psychiatric healthcare preferred. Comprehensive understanding of psychiatric services and hospital capabilities is essential. Familiarity with medical documentation and compliance with regulations is required.

Skills:

Strong customer service and communication abilities (both verbal and written). Capability to prioritize tasks and perform under pressure. Demonstrated critical thinking skills and goal achievement.

Travel Requirements:

Up to 30%
