JOB SUMMARY
Under the guidance of a registered nurse, the Practical Nurse Specialist plays a vital role in the nursing process, which includes assessing, planning, implementing, and evaluating patient care in a home health setting, adhering to established policies and procedures. This position is responsible for assisting the registered nurse in executing care plans for patients across all life stages, including adults and the elderly, while following the organization's policies, procedures, and nursing standards. The role involves delivering nursing care tailored to the needs of patients based on professional experience and competency evaluations.
PRIMARY JOB DUTIES
1. Collaborating in the assessment, planning, implementation, and evaluation of care tailored to the patient's age and medical condition.
2. Efficiently managing patient care in partnership with a multidisciplinary team.
3. Communicating essential information in line with agency policies and professional guidelines.
4. Contributing to the overall effectiveness of care programs.
5. Organizing and executing tasks in a timely and efficient manner.

JOB SPECIFICATIONS
1. Education: Must be a graduate of an accredited or approved practical nursing program.
2. Licensure / Certifications: Must hold a current license to practice practical nursing in the State of North Carolina. CPR certification is mandatory.
3. Experience: A minimum of one year of practical nursing experience is required, with a preference for home health experience.
4. Essential Technical / Motor Skills: Requires hand/eye coordination for administering injections, operating a computer, etc. Proficiency in English communication is necessary. Must be capable of handling patient care equipment and safely transferring patients.
5. Interpersonal Skills: Ability to cultivate positive interactions with patients, their families, physicians, and staff to ensure effective patient care.
6. Essential Physical Requirements: Must be able to lift and maneuver objects weighing at least 50 pounds while providing patient care. The role requires frequent lifting, moving, and positioning of patients, which demands significant physical effort.
7. Essential Mental Abilities: Must possess the ability to assess patient conditions, develop care plans, select appropriate interventions, evaluate patient responses to treatment, and educate patients about their health conditions. This role requires strong cognitive skills, including memory and the ability to prioritize tasks.
8. Essential Sensory Requirements: Ability to visually assess patients and utilize sight for implementing and evaluating care plans. Hearing is essential for auscultating sounds and effectively communicating with patients, families, and staff.
9. Exposure to Hazards: May encounter noise, bloodborne pathogens, and body fluids. Potential exposure to hazardous situations during home visits is possible.
10. Hours of Work: Variable schedule, including weekdays, weekends, and holidays as required.
11. Population Served: The role involves working with adolescents, adults, geriatrics, and pediatrics.
12. A valid North Carolina driver's license and access to a reliable vehicle are required.
