Job Summary:

We are seeking a highly skilled and compassionate LTSS Service Coordinator - RN to join our team at Elevance Health. As a key member of our care management team, you will be responsible for providing exceptional care to our members, ensuring their needs are met, and optimizing their healthcare outcomes.

Key Responsibilities:

  • Conduct thorough clinical assessments to identify member needs and develop personalized care plans.
  • Collaborate with healthcare teams to coordinate care, manage chronic illnesses, and prevent complications.
  • Provide education and support to members and their caregivers to promote healthy behaviors and self-management.
  • Monitor and evaluate member progress, making adjustments to care plans as needed.
  • Communicate effectively with members, families, and healthcare providers to ensure seamless care coordination.

Requirements:

  • Current, unrestricted RN license in applicable state(s)
  • Minimum 3 years of experience in care management, case management, or a similar role
  • Strong clinical skills, with ability to assess and manage complex member needs
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks

Preferred Qualifications:

  • Bilingual (English/Spanish) preferred
  • Experience working with older adults in care management or a similar capacity
  • State-specified certification based on state law and/or contract
