Certified Hospice Care Assistant

The hospice aide is an integral member of the interdisciplinary team who works under the supervision of a registered nurse to provide essential care services to patients. The role involves observing patients, reporting observations, and documenting care provided. To ensure quality, continuity, and safety of patient care, the hospice aide will be assigned accordingly.
Duties and Responsibilities- Provide personal care services, including bathing, back rubs, oral hygiene, and changing bed linen as assigned.
- Assist patients with dressing and undressing as needed.
- Plan and prepare nutritious meals, including shopping, as assigned.
- Assist with feeding patients as assigned.
- Take and record vital signs, including temperature, pulse, respiration, and blood pressure, as ordered.
- Observe and report on patient skin condition to prevent tissue breakdown.
- Communicate patient condition and significant changes to the Case Manager.
- Assist with patient ambulation and exercise as instructed by the hospice nurse or therapist.
- Provide respite care for patients and their families as needed.
- Maintain patient living areas in a clean and orderly condition, as assigned.
- Adhere to organization documentation and care procedures, and standards of personal and professional conduct.
- Participate in quality assessment and performance improvement teams and activities.
- Must have completed a state-established training program or competency evaluation program that meets the requirements of S a) or S b).
- Meet the training requirements of the State.
- Be at least 18 years old.
- Ability to read and follow written instructions and document care given.
- Understand hospice philosophy and be comfortable providing specialized care to the terminally ill.
- Satisfactory references from previous employers.
- Be self-directing with the ability to work with little direct supervision.
- Have empathy for the needs of the ill, injured, frail, and impaired.
- Possess and maintain current CPR certification.
- Demonstrate tact, patience, and good personal hygiene.
- Be a licensed driver with a reliable vehicle and insurance.
- Good oral and written communication skills.
- Good organizational skills.
