Location: Briarwood (Queens)
Salary Range: $50,000.00 - $60,000.00
Role Summary: This position is responsible for delivering counseling, case management, and various services related to chemical dependency to address the social and emotional needs of individuals experiencing chronic homelessness. The role includes coordinating with local agencies to ensure participants maintain permanent housing. Proof of COVID-19 vaccinations is required.
Key Responsibilities:
- Manage an individual caseload of single adult participants.
- Provide supportive counseling services.
- Conduct assessments and treatment for patients enrolled in Chemical Dependency Services, employing current evidence-based practices in addiction treatment.
- Contribute to the formulation of treatment plans and set both short-term and long-term objectives.
- Facilitate connections with community resources, offer daily living skills training, and ensure compliance with program requirements through documentation.
- Incorporate feedback from team discussions into treatment strategies.
- Engage in clinical team meetings to review individual cases and monitor client progress.
- Maintain communication with all relevant agencies to ensure adherence to service plans.
- Keep accurate records, including progress notes, treatment plans, and discharge summaries, ensuring timely submission.
- Identify program needs and provide recommendations for improvement.
- Offer crisis intervention and stabilization support as necessary, which may include after-hours follow-up for emergencies.
- Perform additional tasks as assigned.
Qualifications:
Must possess current NYS certification as a Credentialed Alcoholism and Substance Abuse Counselor. A Bachelor's degree in a Human Services field is preferred, along with two years of experience in a similar environment or with substance abuse, mental health, and homeless populations. Bilingual Spanish is advantageous. Flexibility to work some evenings and occasional weekends is required. Strong writing and oral communication skills are essential. Proficiency in computer software for document preparation, data management, and tracking is necessary. Experience in crisis intervention techniques and strength-based approaches is expected. A valid driver's license and access to a vehicle are required.
