Job Summary

The Patient Services Coordinator - LPN is a critical role within HCR Home Care, responsible for ensuring the smooth operation of clinical services. This position requires a skilled and detail-oriented individual to coordinate patient care, manage scheduling, and maintain accurate records.

Key Responsibilities
  • Scheduling and Coordination: Directly responsible for scheduling patient visits, coordinating client care, and completing tasks necessary for the operation of clinical services.
  • Record Keeping and Reporting: Maintain accurate records in HCHB, including task completion and patient information.
  • Communication and Collaboration: Ensure effective communication with field staff, physicians, and other stakeholders to ensure seamless patient care.
  • Problem-Solving and Decision-Making: Utilize strong analytical skills to resolve scheduling conflicts, address patient concerns, and make informed decisions to ensure optimal patient outcomes.
  • Compliance and Regulatory Requirements: Adhere to all relevant laws, regulations, and agency policies, including HIPAA and Employee Compliance Program.
Requirements
  • Education and Licensure: Graduation from an accredited program of practical nursing education and unexpired/unencumbered NYS LPN licensure required.
  • Skills and Experience: Possess a good understanding of staffing and scheduling requirements related to home health care, excellent communication skills, and strong organizational abilities.
  • Computer and Keyboarding Skills: Proficient in computer and keyboarding skills, with experience in HCHB and other relevant software.
Work Environment

The Patient Services Coordinator - LPN works primarily in an office setting, with occasional exposure to outdoor conditions. The working conditions are classified as light work, requiring the ability to sit for extended periods and perform tasks that involve minimal physical exertion.
