We are seeking a skilled Patient Advocate Nurse to join our team at Foundation Health Services in Fairbanks, Alaska.

Salary Range: $52,000 - $80,000 per year, based on experience and education

About the Role: As a Patient Advocate Nurse, you will be responsible for advocating for patients' needs and ensuring their rights are respected.

Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Collaborate with the healthcare team to ensure effective communication and coordination of care
  • Evaluate patient progress and adjust care plans as needed
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

  • Valid current or temporary Alaska RN license
  • Associate's degree in nursing; Bachelor's degree preferred
  • BLS certification; ACLS, PALS, or additional certification may be required
