Blended Case Manager

We are seeking a highly skilled Blended Case Manager to join our team at the Community Guidance Center. As a Blended Case Manager, you will be responsible for providing trauma-informed care to our consumers, working closely with them to develop and implement personalized service plans.
Key Responsibilities- Assessment and Service Planning: Conduct thorough assessments of consumers' needs and develop service plans that promote growth and change.
- Linking with Services and Gaining Access: Assist consumers in locating and accessing necessary services, meeting them where they need it most.
- Monitoring Service Delivery: Review services in which consumers are involved, ensuring they are receiving the support they need.
- Problem Resolution: Assist consumers in addressing problems with services, advocating on their behalf when necessary.
- Informal Support Network Building: Establish and maintain relationships with consumers' families, with their permission.
- Use of Community Resources: Help consumers identify and access community resources, assessing their ability to use them effectively.
- Bachelor's degree in a relevant field, such as sociology, social work, or psychology.
- Act 33 & 34 Clearances and FBI Clearance.
- Computer skills and ability to work at a desk for intermittent periods.
- Command of the English language, including speaking, understanding, and writing.
- Good public speaking skills for small and large groups.
- Solid organizational skills and time-management skills.
- Ability to tolerate moderate to high stress levels in crisis situations.
- Valid driver's license and vehicle for attending consumer/agency appointments.
The Community Guidance Center is a private, non-profit mental and behavioral health center serving communities in Indiana and Pennsylvania. We provide a wide range of therapeutic and rehabilitative services, and are certified as a Sanctuary Institute organization for trauma-informed care.
