Job Title: Children's Blended Case Manager

Job Summary:
Mitchell Martin is seeking a skilled Children's Blended Case Manager to join our team. The ideal candidate will have a strong background in social work, psychology, or a related field, with experience in public or private human services. The successful candidate will be responsible for providing case management services to families, linking them to program services, treatment, and community supports. This role requires a strong understanding of crisis intervention, the Philadelphia Behavioral Health and Juvenile System, and community resources.

Key Responsibilities:
Conduct functional assessments to identify skill deficits and strengths for each consumer on the caseload.
Develop and implement written service plans for each consumer, ensuring they receive the necessary support to achieve self-sufficiency.
Identify and connect consumers to appropriate clinical and community resources, including behavioral health and social service agencies.
Provide 24-hour response to consumers in crisis, including wearing a company-issued cell phone.
Participate in interagency team meetings and make home, school, hospital, and community-based visits as needed.
Other responsibilities as designated by the supervisor.

Requirements:
Bachelor's Degree in sociology, social work, psychology, gerontology, or other social science.
Two years of experience in public or private human services, with one year of direct client contact required.
Masters Degree preferred.
Bilingual (Spanish/English) preferred.
Working knowledge of Crisis Intervention, the Philadelphia Behavioral Health and Juvenile System, and Community Resources required.

Preferred Skills:
Bilingual (Spanish/English) a plus.
Decision Making, Communication, Time Management, Organizational Skills, Leadership, and Technical Capacity required.
Intermediate skills in Microsoft Office required.
Ethical Conduct required.
