Diabetes Educator Job Opportunity

We are seeking an experienced Diabetes Educator to join our Pediatric Diabetes Education Program. As a key member of our multidisciplinary team, you will play a critical role in providing high-quality patient education and support services.

About the Role:

The successful candidate will have a strong background in diabetes care, excellent communication skills, and the ability to work effectively with diverse populations. You will be responsible for developing and implementing individualized care plans, conducting educational sessions, and collaborating with interdisciplinary teams to ensure seamless care delivery.

Key Responsibilities:

  1. Conduct educational sessions with patients and their families, focusing on disease management, self-care, and wellness.
  2. Develop and implement individualized care plans, taking into account each patient's unique needs and goals.
  3. Collaborate with interdisciplinary teams, including physicians, nurses, dietitians, and social workers, to ensure comprehensive care delivery.
  4. Document patient progress, identifying areas for improvement and adjusting care plans as needed.
  5. Communicate effectively with patients, families, and healthcare teams, ensuring clear understanding of treatment plans and expectations.

Requirements:

  • RN license in New Mexico or reciprocal agreement by NM.
  • CPR for Healthcare/BLS Prov or Prof Rescuers certification within 30 days.
  • Bilingual English, Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or Navajo (preferred).
  • 3 years directly related experience.

Working Conditions:

  • Medium work: Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects or people.
  • Mod exposure to chemicals/dangerous equip/materials.
