Clinical Nurse Specialist

As a Registered Nurse (RN) at Fenway Health, you will deliver empathetic, evidence-informed, and effective healthcare to individuals, families, and communities. The role of the Registered Nurse aligns with the standards set by the Massachusetts Board of Registration in Nursing, the ANA Scope and Standards of Practice, and the ANA Code of Ethics. The RN is expected to fulfill professional obligations and actively engage in leadership to foster a collaborative, innovative, and educational environment.
The RN will ensure high-quality nursing care that reflects a commitment to patient-centered approaches, inclusivity, harm reduction, and integrated behavioral health. This position involves clinical leadership responsibilities, focusing on disease management, care coordination, and triage for a designated patient panel. The RN will exemplify nursing excellence and professionalism, consistently upholding the mission of Fenway Health to enhance the health and well-being of the LGBTQIA+ community and all individuals in our neighborhoods.

Key Responsibilities
- Collaborate with healthcare providers and team members to enhance patient outcomes through tailored care plans that promote health and prevent illness.
- Deliver competent and compassionate nursing care grounded in clinical reasoning and patient experiences.
- Conduct assessments and triage via virtual, telephonic, or in-person methods to evaluate patient health concerns.
- Implement and monitor patient-centered care plans, ensuring effective communication with patients and care teams.
- Facilitate the efficient use of healthcare resources by coordinating care and encouraging patient participation in wellness initiatives.
- Document patient information accurately and in a timely manner within the electronic health record (EHR).

Qualifications
- Education: Associate degree in nursing.
- Current Massachusetts Registered Nurse (RN) license.
- Current CPR certification.
- Strong interpersonal and communication skills.
- Experience in diverse healthcare settings.
- Interest in LGBTQIA+ health and community service.
Preferred Qualifications
- Bachelor's degree in nursing.
- Two years of experience in various nursing environments.
- Experience in complex healthcare systems.
Working Conditions
