Responsibilities:
The Community Engagement Coordinator will focus on outreach to eligible patients with the aim of enrolling them in HarmonyCares services.
This role involves providing detailed explanations of services, addressing a variety of inquiries, gathering feedback, and scheduling appointments for new patients.
The Coordinator will serve as the local market leader in B2B, B2C, and DTC engagements, representing our community of referral partnerships.
The team will be tasked with fostering relationships with community Primary Care Providers (PCPs) and Specialists with whom we share patients.
Additionally, the Coordinator will enhance patient and family engagement through direct outreach via phone and community interactions.
Key Duties and Responsibilities:- Identify innovative methods to enroll patients into HarmonyCares
- Cultivate relationships with Internal Medicine and Family Practices, Community Specialists, local hospitals, home health agencies, skilled nursing facilities, and clinics to enhance engagement of eligible patients
- Schedule regular meetings with community partners to discuss HarmonyCares services, current patients/families, and potential candidates
- Respond promptly to inquiries while meeting the needs of patients and HarmonyCares internal team members
- Build collaborative relationships with practice Managers, PCPs, and key provider stakeholders to establish formal business connections
- Conduct telephonic outreach to eligible patients or families to facilitate enrollment in the HarmonyCares program
- Assist in coordinating and facilitating the enrollment and onboarding of new patients, including managing patient consents, appointment setups, and visit reminders
- Utilize reporting and dashboards to prioritize outreach efforts to members
- Achieve or surpass established monthly engagement targets
- Perform other duties as assigned
Qualifications:
Required Knowledge, Skills, and Experience
- High school diploma
- 2+ years of relevant experience in sales, direct marketing, or customer service within the medical/healthcare sector
- Proficient in Microsoft Excel, Word, and PowerPoint
- Access to reliable transportation
- Valid driver's license and a clean driving record for non-remote employees
- Ability to guide patients and families through the enrollment process
- Strong communication skills with an empathetic, compassionate, and professional demeanor
- Proven problem-solving abilities with a capacity to tackle complex issues effectively and efficiently
- Analytical skills to assess problems and resolve issues promptly and thoroughly
Preferred Knowledge, Skills, and Experience
- Associate's degree
- Experience in provider engagement
- Previous healthcare sales experience
- Familiarity with CRM platforms
- Understanding of the healthcare industry
- Proficient in PowerPoint and Excel
- Dynamic personality with a talent for building strong interpersonal relationships
- Comfort with medical terminology
- Ability to navigate various medical environments, including hospitals, doctors' offices, and pharmacies
- Familiarity with different types of healthcare providers (e.g., Physicians, Physician Assistants, Nurse Practitioners)
