We are seeking a highly skilled and experienced Social Services Director to join our team at the Cranbury Center in Monroe Township, NJ. As a key member of our leadership team, you will be responsible for overseeing the operation of our Social Services Department, ensuring compliance with federal, state, and local guidelines and regulations, and fostering a climate that promotes patient/resident dignity and well-being.

Key Responsibilities:
  • Supervise and manage a team of Social Services staff, providing guidance and support to ensure high-quality patient/resident care.
  • Develop and implement policies and procedures to ensure compliance with regulatory requirements and promote best practices in social services.
  • Conduct psychosocial assessments to identify patient/resident needs and strengths, and develop individualized care plans to address these needs.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and promote patient/resident-centered care.
  • Monitor and evaluate departmental productivity and outcomes, making data-driven decisions to improve services and quality of care.
Requirements:
  • Bachelor's degree in social work or human services required.
  • Must possess any certifications/licensures as required by state of employment to practice in long-term care.
