We are seeking a highly skilled and experienced Social Services Director to join our team at Cranbury Center in Monroe Township, NJ. As a key member of our leadership team, you will be responsible for overseeing the operation of our Social Services Department, ensuring compliance with federal, state, and local guidelines and regulations, and fostering a climate that promotes patient/resident dignity and well-being.
Key Responsibilities:- Supervise and manage a team of Social Services staff, providing guidance and support to ensure excellent patient/resident care.
- Develop and implement policies and procedures to ensure compliance with regulatory requirements and promote a culture of excellence.
- Conduct psychosocial assessments to identify patient/resident needs and strengths, and develop individualized care plans to address these needs.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and promote patient/resident dignity and well-being.
- Monitor and evaluate departmental productivity and outcomes, making data-driven decisions to improve performance.
- Bachelor's degree in social work or human services required.
- Must possess any certifications/licensures as required by state of employment to practice in long-term care.
