We are seeking a highly skilled and experienced Social Services Director to join our team at Cranbury Center in Monroe Township, NJ. As a key member of our leadership team, you will be responsible for overseeing the operation of our Social Services Department, ensuring compliance with federal, state, and local guidelines and regulations, and fostering a climate that promotes patient/resident dignity and well-being.

Key Responsibilities:
  • Supervise and manage a team of Social Services staff, ensuring effective delivery of services and high-quality care.
  • Develop and implement policies, procedures, and standards of care that align with the National Association of Social Workers (NASW) Code of Ethics.
  • Conduct psychosocial assessments to identify patient/resident needs and strengths, and develop discharge plans that promote independence and dignity.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients/residents and their families.
  • Monitor and evaluate departmental productivity, outcomes, and quality of care, making recommendations for improvement.
Requirements:
  • Bachelor's degree in social work or human services required.
  • Must possess any certifications/licensures as required by state of employment to practice in long-term care.
