We are seeking a highly skilled and experienced Social Services Director to join our team at Cranbury Center in Monroe Township, NJ. As a key member of our leadership team, you will be responsible for overseeing the operation of our Social Services Department, ensuring compliance with federal, state, and local guidelines and regulations, and fostering a climate that promotes the well-being and dignity of our patients/residents.

Key Responsibilities:
  • Supervise and manage a team of Social Services staff, ensuring they have the necessary skills and resources to provide high-quality care.
  • Develop and implement policies and procedures that promote patient/resident-centered care and ensure compliance with regulatory requirements.
  • Conduct psychosocial assessments to identify social, emotional, and psychological needs and strengths of patients/residents.
  • Collaborate with other departments to ensure seamless care transitions and effective communication.
  • Stay up-to-date with industry developments and best practices, applying this knowledge to improve our services and outcomes.
Requirements:
  • Bachelor's degree in social work or human services required.
  • Must possess any certifications/licensures as required by state of employment to practice in long-term care.
