About the Role

We are seeking a skilled Mental Health Professional to join our team at Care New England Health System. As a Patient Advocate and Care Coordinator, you will play a vital role in providing exceptional care to our patients and their families.

Key Responsibilities
  • Assist registered nurses in the direct care of patients, focusing on activities of daily living and ensuring a safe and therapeutic environment on the unit.
  • Communicate and meet patients' needs for service, promoting a patient-centered approach to care.
  • Participate in groups and provide support to patients, promoting a sense of community and well-being.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Provide education and support to patients and their families, promoting health literacy and self-advocacy.
Requirements
  • Bachelor's degree in a social science or a related field (or equivalent experience).
  • Experience working with mentally ill and/or developmentally delayed clients.
  • Excellent written and communication skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
  • Strong problem-solving and critical thinking skills.
