Position Overview

PeaceHealth is in search of a Clinical Pharmacist for a Full Time, 1.00 FTE, Variable role. The compensation range for this position is $97.18. The starting salary is influenced by various factors such as education, training, work experience, and applicable collective bargaining agreements.

Role Responsibilities

This role is pivotal in ensuring the effective utilization of medications to enhance patient outcomes and improve quality of life. Key responsibilities include both cognitive pharmaceutical care and medication distribution, which encompass:
  • Dispensing and distributing medications.
  • Providing guidance to healthcare providers and patients on the correct use of prescribed medications.
  • Collaborating with other healthcare professionals to plan, monitor, review, and assess patient treatment effectiveness.
  • Monitoring patients for potential side effects and drug interactions.

Key Duties
  • Assess medication orders for accuracy and appropriateness, including contraindications, allergies, dosage, route, frequency, and formulary status.
  • Clarify and correct physician medication and IV orders, including medication reconciliation.
  • Evaluate outpatient prescriptions and select appropriate medications for dispensing.
  • Monitor patients on specific target medications and initiate pharmacokinetic dosing protocols.
  • Review drug therapy for suitability and provide recommendations to clinicians.
  • Prescribe medications in accordance with collaborative drug therapy management agreements.
  • Dispense medications for administration and conduct patient counseling.
  • Identify, monitor, and report adverse drug reactions and medication errors.
  • Oversee and train pharmacy technicians to ensure accurate order entry and preparation.
  • Supervise pharmacy interns and externs, potentially serving as a preceptor.
  • Contribute to the development of drug protocols and clinical pathways.
  • Conduct drug usage evaluations and report findings to the Pharmacy & Therapeutics Committee.
  • Complete quality assurance audits as required for prescriptive authority.
  • Perform additional duties as assigned.

Qualifications
  • Bachelor's Degree in Pharmacy required.
  • Doctor of Pharmacy from an accredited institution required.
  • Demonstrated foundational pharmacy clinical experience required.
  • Current Oregon Pharmacist license or eligibility to obtain one required.
