Position Overview:

Deliver comprehensive pharmaceutical care to patients, focusing on clinical oversight and therapeutic strategies that enhance patient outcomes. Ensure precision in dispensing and oversee pharmacy operations to guarantee accurate and prompt medication distribution.

Key Responsibilities:

Medication Order Review:

Scrutinize and interpret medication orders from licensed prescribers or their representatives, ensuring compliance with the standards of a complete medication order. Verify that the prescribed medication and dosage are suitable for the patient's condition. Evaluate potential drug interactions, allergy alerts, and duplication of therapy.

Seek clarification from the prescriber for any incomplete medication orders or those that may lead to suboptimal therapeutic outcomes.

Provide prescribers with suitable therapeutic alternatives. Conduct a secondary verification of medication orders for pediatric patients and chemotherapy prescriptions.


Clinical Therapeutic Monitoring - Actively engage in the continuous assessment of medication therapy in accordance with established Collaborative Care Agreements and medication management protocols.

Guide medication therapy within the parameters set by Collaborative Care agreements and protocols. Communicate with prescribers as necessary to propose therapeutic alternatives that optimize patient care.

Document medication therapy evaluations and recommendations in the patient's medical record using iVents and progress notes, keeping the interdisciplinary team informed of pharmacy-related assessments and interventions.



Compounding and Dispensing Medications:
Ensure the timely dispensing of the correct medication quantities.

Oversee compounding in line with USP standards, verifying ingredient compatibility, content accuracy, and appropriate labeling, including expiration dates.

Review all medications prior to final dispensing for direct patient administration, distribution via automated dispensing systems, and floor stock dispensing.

Utilize barcode technology whenever feasible to ensure precise dispensing.

Supervise the repackaging of medications, including verification and documentation of lot numbers, expiration dates, and the identities of individuals involved in the process.


Supervision of Interns and Technicians: Provide direct supervision and mentorship to Interns and Technicians, guided by ICARE values.

This includes monitoring the timely completion of tasks, redirecting staff to meet patient needs, and promoting the use of the AIDET model in interactions with staff, patients, and visitors.

Report behaviors that do not respond to direct intervention to the leadership team.

Medication Safety Initiatives:

Identify and report variances in dispensing promptly using internal variance report forms or Midas as appropriate.

Participate in departmental efforts to enhance the safe use of medications (e.g., clear labeling, new SALA medications, and Epic improvements/corrections). Report suspected adverse drug reactions in Midas.

Controlled Substance Management:

Engage in and oversee the receipt, dispensing, and distribution of controlled substances utilizing Pyxis C-II Safe, Pyxis Medstation 4000, internal perpetual inventories, Pyxis Anesthesia Stations, Anesthesia Boxes, and anesthesia waste logs.

Ensure inventory accuracy, reconcile anesthesia waste, and assist in resolving controlled substance inventory discrepancies, including providing Pyxis activity reports to nursing.

Reconcile the Pyxis vs. CII Safe report to ensure the proper handling of controlled substances to Pyxis Medstations.

Contribute to the Financial Health of the Department: Encourage prescriber adherence to formulary restrictions and therapeutic interchanges. Ensure formulary compliance and process non-formulary requests appropriately. Suggest suitable therapeutic alternatives for non-formulary requests. Actively participate in defined IV to PO conversions. Utilize time efficiently, delegating work when appropriate to minimize overtime.

Drug Information and Staff Development:
Provide accurate, timely drug information to medical staff, nursing staff, and patients. Serve as a mentor or advisor to Pharmacy Residents and Pharmacy Interns. Develop educational programs for staff.

Maintain Professional Competence:
Uphold licensure. Complete all departmental competency and skills assessment requirements. Stay informed about current pharmacy practices. Seek to develop and maintain expertise in specialized practice areas.

Perform additional duties as assigned.

Qualifications:

Education, Credentials, Licenses:

Graduate from an ACPE accredited pharmacy program

Current pharmacist license in the state of practice

Specialized Knowledge:

Strong problem-solving and critical thinking abilities

Effective communication skills

Familiarity with Current Practice Standards (ASHP)

Experience:

Prior hospital experience
