Position Overview:

Deliver comprehensive pharmaceutical care to patients, focusing on clinical oversight and therapeutic interventions aimed at enhancing patient outcomes. Provide educational resources and programs to customers. Support the ongoing enhancement of Performance Improvement initiatives to refine our Medication Use Process. Ensure precision in dispensing and oversee pharmacy operations to guarantee timely and accurate medication distribution.

Key Responsibilities:

Medication Order Verification:

Analyze and interpret medication orders from licensed prescribers, ensuring compliance with the standards of a complete medication order. Verify that the prescribed medication and dosage are suitable for the patient's condition. Evaluate potential drug interactions, allergy alerts, and duplication of therapy.

Seek clarification from prescribers for any incomplete orders or those that may lead to suboptimal therapeutic outcomes. Provide prescribers with suitable therapeutic alternatives. Conduct a secondary verification of medication orders for pediatric patients and chemotherapy prescriptions.



Therapeutic Clinical Monitoring:

Actively engage in the continuous monitoring of medication therapy in accordance with established Collaborative Care Agreements and medication management protocols. Guide medication therapy within the parameters defined by these agreements and protocols. Communicate with prescribers as necessary to optimize patient care.

Document medication therapy evaluations and recommendations in the patient's medical record, utilizing iVents and progress notes to keep the interdisciplinary team informed of pharmacy-related assessments and interventions.



Compounding and Dispensing:

Ensure the prompt dispensing of the correct medication quantities. Supervise compounding processes in line with USP standards, verifying ingredient compatibility, content accuracy, and appropriate labeling, including expiration dates.

Conduct thorough checks of all medications prior to final dispensing for direct patient administration, automated dispensing cabinets, and floor stock. Utilize barcode technology whenever feasible to guarantee accurate dispensing.



Supervision of Interns and Technicians:

Provide direct supervision and mentorship to interns and technicians, adhering to ICARE values. This includes monitoring task completion, redirecting staff to meet patient needs, and promoting the AIDET model in interactions with staff, patients, and visitors. Report any behaviors that do not respond to direct intervention to the leadership team.



Medication Safety Initiatives:

Identify and report variances in dispensing promptly using internal variance report forms or Midas as appropriate. Participate in departmental initiatives aimed at enhancing the safe use of medications, such as clear labeling and improvements in Epic. Report suspected adverse drug reactions in Midas.



Controlled Substance Management:

Participate in and oversee the receipt, dispensing, and distribution of controlled substances using systems such as Pyxis C-II Safe and Pyxis Medstation 4000. Ensure accurate inventories, reconcile anesthesia waste, and assist in resolving discrepancies in controlled substance inventories.



Financial Contributions:

Encourage prescriber adherence to formulary restrictions and therapeutic interchanges. Ensure formulary compliance and process non-formulary requests appropriately. Suggest suitable therapeutic alternatives for non-formulary requests and actively engage in defined IV to PO conversions. Optimize time management to minimize overtime.



Drug Information and Staff Development:

Provide timely and accurate drug information to medical and nursing staff, as well as patients. Serve as a mentor or advisor to Pharmacy Residents and Interns. Develop educational programs for staff.



Professional Development:

Maintain licensure and complete all competency assessments required by the department. Stay informed about current pharmacy practices and seek to enhance skills in specialized areas.



Perform additional duties as assigned.

Qualifications:

Education and Credentials:

Graduate from an ACPE accredited pharmacy program

Current pharmacist license in the state of practice

Specialized Knowledge:

Strong problem-solving and critical thinking abilities

Effective communication skills

Familiarity with Current Practice Standards (ASHP)

Experience:

Prior hospital experience
