Home Health LPN

At CenterWell Home Health, we're dedicated to providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. As a Home Health LPN, you'll play a vital role in our care teams, working alongside nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.
Key Responsibilities:- Provide professional nursing services under the direction of a RN in compliance with state laws and regulations, healthcare standards, and CenterWell Home Health policies and procedures.
- Deliver skilled nursing interventions in the treatment of patients' illnesses, rehabilitative needs, and preventative care, utilizing a holistic approach to problem-specific therapeutic interventions, teaching, and training activities in accordance with the plan of care.
- Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
- Maintain contact with patients, physicians, clinical managers, and other healthcare team members in a timely manner regarding patient/family needs and status changes, participating in care coordination activities and discharge planning as appropriate.
- Ensure adherence to professional standards in the provision of and availability of supplies, materials, and equipment needed to safely and effectively implement the plan of care.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current IV Certification.
- Per Diem = Available weekdays.
- Two years experience as an LPN/LVN in a clinical setting; Skilled Home Health experience required for per diem schedule.
