LPN, Home Health Care Specialist

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health Care Specialist. As a key member of our care team, you will provide high-quality nursing services to patients in their homes, promoting independence, dignity, and overall well-being.
Responsibilities- Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans to address patients' physical, emotional, and social needs.
- Conduct thorough assessments, identify health risks, and develop strategies to prevent complications.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Document patient progress, outcomes, and any changes in condition, maintaining accurate and timely records.
- Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
