LPN for Skilled Home Visits

We are seeking a skilled and compassionate LPN to join our team at CenterWell Home Health. As a Home Health Nurse, you will play a vital role in providing high-quality care to patients in their homes, enabling them to recover and regain their independence.
Key Responsibilities- Provide professional nursing services under the direction of a RN, in compliance with state and federal regulations, and CenterWell Home Health policies and procedures.
- Develop and implement individualized plans of care, utilizing a holistic approach to address the physical, emotional, and social needs of patients.
- Confer with RN Supervisors to ensure seamless care coordination and make necessary changes to the Plan of Treatment.
- Maintain accurate and timely documentation, including visit/shift notes and other required records.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients and their families.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current IV Certification.
- Two years of experience as an LPN/LVN in a clinical setting, with Skilled Home Health experience preferred.
