Key Responsibilities- Provide skilled nursing interventions to patients in their homes, under the direction of a RN.
- Confer with the RN Supervisor to ensure seamless care coordination.
- Maintain open communication with patients and their families to ensure their needs are met.
- Prepare accurate and timely documentation of patient care.
To be successful in this role, you will need:
- A graduate degree from an accredited LPN program.
- A current nursing license.
- IV certification.
- At least two years of experience as an LPN in a clinical setting.
