The Behavioral Health Home (BHH) division at Woodfords Family Services is in search of skilled BHH Coordinators (Children's Case Managers) to enhance our dedicated team of professionals.

These roles are full-time (32-40 hours per week) and are based in Maine.


Role Summary

The BHH Coordinator plays a crucial role in supporting children with disabilities and their families, providing advocacy, guidance, and access to services that foster family empowerment and independence. This is accomplished through various methods including: comprehensive assessments, care plan development, ongoing evaluation, and facilitating family/team meetings.

Key Responsibilities:

  • Conduct regular meetings with children and families in diverse environments to evaluate functioning, progress, quality of life, and safety concerns.
  • Possess an understanding of community resources and assist families in accessing these services.
  • Perform strength and needs assessments for all children and coordinate family wrap-around meetings as necessary.
  • Act as a liaison with other service providers (public schools, behavioral health agencies, etc.) to ensure adequate care and address any service gaps.

BHH Coordinators benefit from reduced productivity expectations AND additional incentives starting at $350


Work Schedule & Compensation

Full-time, 32-40 hours per week

Monday through Friday - Flexible hours and scheduling

Annual salary between $44,000 and $46,000 (based on experience)

$1,500 Sign-on Bonus disbursed after 6 months of employment

Qualifications

  1. Bachelor's Degree in Social Work or a related Human Services field (LSW Preferred).
  2. A minimum of three years of experience in the field.
  3. Familiarity with regulatory guidelines.
  4. Valid driver's license and access to a vehicle.
