Make a Difference in the Lives of Children and Families

Woodfords Family Services is seeking qualified Behavioral Health Home Coordinators (Children's Case Managers) to join our team of dedicated professionals. As a BHH Coordinator, you will work with children with disabilities and their families to provide support, advocacy, and connection to services that promote family empowerment and self-sufficiency.

Key Responsibilities:
  • Meet regularly with child and family to assess consumer functioning, progress, quality of life, and health and safety issues.
  • Possess knowledge regarding community resources and assist families with referrals to such resources.
  • Conduct strength and needs assessments for all children and facilitate family wrap-around meetings as needed.
  • Serve as a liaison with other agencies involved in providing other types of services to the child.
What We Offer:
  • Full-time, 32-40 hours per week
  • Monday through Friday - Flexible hours & schedule
  • $44,000-$46,000 annual salary (commensurate with experience)
  • $1,500 Sign-on Bonus Paid at 6 months of employment

Qualifications:
  • Bachelor's Degree in Social Work or related Human Service field (LSW Preferred)
  • At least 3 years experience in the field
  • Knowledgeable about regulatory guidelines
  • Valid driver's license and use of a vehicle
