Admissions Nurse or Admissions Coordinator

We are seeking a skilled Admissions Nurse or Admissions Coordinator to join our team at Foundations for Living. This dynamic role involves assessing potential patients for admission and working closely with healthcare professionals to provide high-quality care.
Key Responsibilities- Conduct thorough assessments of patients to determine their suitability for admission
- Collaborate with physicians, nurse practitioners, and other healthcare professionals to develop individualized care plans
- Screen and assess patients in various settings, including triage, scheduled assessments, and crisis management
- Work closely with nursing staff to ensure seamless transitions of care
- Participate in quality improvement initiatives to enhance patient outcomes
- Registered Nurse licensure in the state of Idaho or a master's level Social Worker licensure
- Training in Crisis Prevention Instruction and CPR
- Preferred experience in psychiatric settings
