Job Title: Social Worker

Mount Sinai Health System is seeking a skilled Social Worker to join our team. As a Social Worker, you will play a critical role in providing comprehensive social work services to patients and their families.

Responsibilities:
  • Engage patients and their families to assess and evaluate psychosocial needs for discharge planning and/or care management.
  • Formulate interventions, discharge and/or comprehensive care plans that address identified needs.
  • Provide clinical support to achieve mutually identified goals, in accordance with social work practice standards.
  • Connect patients and their families with appropriate community resources.
  • Assess patterns of need within populations and work collaboratively to address them.
Requirements:
  • Master's degree in social work from an accredited institution.
  • Knowledge of social work services theories and techniques.
  • Level of knowledge of social work process and community resources and agencies in relation to health, illness, and disability, generally acquired through six months to one year related experience in a medical or healthcare setting.
