ROLE SUMMARY AND REQUIREMENTS:

The Service Coordinator is tasked with optimizing the use of community resources by residents participating in Volunteers of America Mid-States' housing initiatives for the elderly or disabled. This role is pivotal in enhancing residents' quality of life and ensuring their sustained engagement within the housing programs.

QUALIFICATIONS FOR THIS ROLE:

A Bachelor’s degree in Social Work or a related field such as Gerontology, Psychology, or Counseling is required.

Candidates should possess a solid understanding of the aging process, elder services, disability services, and the eligibility criteria for Federal and State entitlement programs. Knowledge of legal issues, substance abuse, poverty, and mental health challenges is also essential. A minimum of two years of experience in social service delivery to senior citizens or non-elderly disabled individuals is preferred.

KEY RESPONSIBILITIES:

1. Conduct assessments for residents seeking services.

2. Create and update case plans based on assessments; maintain accurate records of progress in case notes after each interaction with residents or their families.

3. Identify and cultivate relationships with community resources and other social service providers.

4. Oversee the ongoing provision of services and keep providers informed of residents' progress as necessary.

5. Educate residents about service availability, application processes, and clients' rights; provide advocacy as required.

6. Develop and maintain a comprehensive directory of service providers for staff and residents.

7. Prepare and submit HUD and other statistical reports accurately and promptly.

8. Organize and execute special projects aimed at enhancing social networks and support systems among residents.

9. Accurately represent agency programs and policies through community engagement.

10. Utilize volunteers to support programs when appropriate.

11. Attend scheduled supervision and training sessions.

12. Perform quality improvement duties as assigned by supervision and the PQI committee.

13. Participate in relevant teleconferences and meetings as designated by the HUD QA Administrator.

14. Engage actively in HUD QA activities throughout the year, with insights shared with the supervisor as part of the performance review process.
