Job Title: Licensed Vocational Nurse - Home HealthJob Summary:

We are seeking a highly skilled and compassionate Licensed Vocational Nurse (LVN) to join our Home Health team at Sutter Health. As an LVN, you will provide high-quality nursing care to patients in their homes, promoting independence and improving health outcomes.

Responsibilities:
  • Provide nursing care to patients in their homes, under the supervision of a registered nurse (RN) or physician.
  • Assess patient needs and develop individualized care plans to meet their unique needs.
  • Administer medications, perform wound care, and provide other nursing interventions as needed.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Document patient progress and maintain accurate records.
Requirements:
  • Graduate of an accredited school of vocational nursing.
  • Current LVN licensure in the state of California.
  • Basic Life Support (BLS) certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
Working Conditions:

This position requires the ability to work in a fast-paced environment and to prioritize tasks effectively. The LVN will be required to travel to patients' homes and may be exposed to various environmental conditions.
