Personal Care Assistant

As a Home Health Aide (HHA), you will deliver essential personal care and supportive services within the client's residence, following the directives set forth by the RN Case Manager. The tasks assigned to you will be based on the established care plan, ensuring that all services are executed under the supervision of a registered nurse who is accountable for the overall care provided.
Key Responsibilities
- Adhere to the care plan to ensure safe and effective service delivery to clients.
- Assist clients in maintaining personal hygiene and a secure living environment.
- Plan and prepare balanced and nutritious meals.
- Support clients with mobility and ambulation as per the approved care plan.
- Encourage client independence in daily activities as outlined in the care plan.
- Promote mental engagement through activities tailored to the client's interests.
- Provide emotional and psychological support to clients and their families, fostering trust and confidentiality.
- Monitor and report any changes in the client's physical or mental health to the supervising nurse.
- Perform light housekeeping duties to maintain a comfortable environment for the client.
- Document progress notes in a timely manner.
- Confirm weekly visit schedules to facilitate coordination with other healthcare staff.
- Collaborate with community agency personnel involved in the client's care.
- Participate in required training sessions as mandated by state regulations.
- Communicate with the RN Case Manager regarding the client's health status and care needs.
- Promptly report any significant changes in the client's condition to the RN Case Manager.
- Operate within the defined scope of practice for Home Aides.
- Must possess adequate hearing, vision, and physical dexterity to perform client assessments and care.
- Occasional lifting and moving of clients, with the ability to carry up to 30 pounds.
- Effective communication skills, including verbal, written, and visual methods.
- Compliance with company policies and guidelines as outlined in the employment contract.

Qualifications
- High school diploma or equivalent required.
- Completion of state-mandated training hours, if applicable.
- Proficient in computer and internet applications for documentation purposes.
- Strong problem-solving abilities and capacity to manage multiple tasks.
- Demonstrated compassion and a caring attitude.
- Knowledge of client care equipment and safety practices.
- Successful completion of a criminal background check.
- Professional appearance is essential.
- Must be in good health, free from conditions that could pose risks to clients or caregivers, with appropriate health documentation.
- Understanding of personal care, housekeeping, and meal preparation principles.
- Respect for client confidentiality and ethical considerations.
