Position Overview

As a Home Health Aide (HHA), you will deliver essential personal care and supportive services within the client's residence, following the directives of the RN Case Manager. Your responsibilities will be defined by the care plan, ensuring that all tasks are executed under the supervision of a registered nurse who oversees the quality of care provided. It is important to note that HHAs are not permitted to adjust oxygen concentrators, manage intravenous procedures, or perform any invasive tasks beyond basic care measures such as rectal temperatures or enemas.

Key Responsibilities

  • Adhere to the established care plan to deliver safe and effective services to clients.
  • Assist clients in maintaining personal hygiene and a secure living environment.
  • Prepare and serve nutritious meals tailored to client needs.
  • Support clients with mobility and ambulation as outlined in the care plan.
  • Encourage client independence in daily activities.
  • Engage clients in activities that promote mental stimulation and emotional well-being.
  • Provide basic emotional support to clients and their families, fostering trust and confidentiality.
  • Monitor and report any changes in the client's physical or mental health to the supervising nurse.
  • Perform light housekeeping duties to ensure a comfortable environment for clients.
  • Document care provided through timely progress notes.
  • Coordinate weekly visit schedules to optimize care delivery.
  • Collaborate with other community service providers involved in the client's care.
  • Participate in required training sessions as mandated by state regulations.
  • Communicate regularly with the RN Case Manager regarding client progress and care needs.
  • Promptly notify the RN Case Manager of any significant changes in client condition.
  • Operate within the defined scope of practice for Home Health Aides.

Job Conditions / Requirements

  • Must possess adequate hearing, vision, and physical dexterity to perform client assessments and care tasks.
  • Occasional lifting and moving of clients may be required, with the ability to lift up to 30 pounds.
  • Effective communication skills are essential, including verbal, written, and visual methods.
  • Compliance with company policies and guidelines is mandatory.
