Position Overview

As a Home Health Aide (HHA), you will deliver essential personal care and supportive services within the client's residence, following the directives of the RN Case Manager. Your responsibilities will be guided by the established care plan, ensuring that all tasks are performed under the supervision of a registered nurse, who holds accountability for the care provided. It is important to note that HHAs are not permitted to adjust oxygen settings, administer intravenous treatments, or perform any invasive procedures, except for basic tasks such as taking rectal temperatures or administering enemas.

Key Responsibilities

  • Adhere to the care plan to deliver safe and effective care to clients.
  • Assist clients in maintaining personal hygiene and a secure living environment.
  • Plan and prepare healthy meals tailored to client needs.
  • Support clients with mobility as outlined in the care plan.
  • Encourage client independence through engagement in daily activities.
  • Facilitate mental stimulation by involving clients in their interests.
  • Provide emotional and psychological support to clients and their families, fostering trust and confidentiality.
  • Monitor and report any changes in the client's physical or mental health to the supervising nurse.
  • Perform light housekeeping duties to ensure a comfortable environment for clients.
  • Document care activities and client progress in a timely manner.
  • Coordinate weekly visit schedules to optimize care delivery.
  • Collaborate with other healthcare professionals involved in the client's care.
  • Participate in required training sessions as mandated by state regulations.
  • Communicate effectively with the RN Case Manager regarding client status and care needs.
  • Promptly inform the RN Case Manager of any significant changes in client condition.
  • Operate within the defined scope of practice for Home Health Aides.

Job Conditions / Requirements

  • Must possess adequate hearing, vision, and physical dexterity to conduct assessments and provide care.
  • Occasional lifting and moving of clients, with the ability to carry up to 30 pounds.
  • Effective communication skills, including verbal, written, and visual methods.
  • Compliance with company policies and guidelines as outlined in the employment contract.
