Job Summary

Heinzerling Community is seeking a dedicated Care Coordinator to enhance the lives of individuals with developmental disabilities.

This role involves preparing and presenting annual Individual Plans (IPs) and quarterly reviews, developing and monitoring skill development programs, and maintaining comprehensive progress notes.

The ideal candidate will have a Bachelor's degree and three years of experience working with individuals with developmental disabilities, strong communication skills, and a valid driver's license.

Key Responsibilities:

  • Prepare and present annual Individual Plans (IPs) and quarterly reviews.
  • Develop and monitor skill development programs in collaboration with interdisciplinary teams.
  • Maintain comprehensive progress notes and conduct assessments.
  • Coordinate and participate in community outings and leisure activities.
  • Train staff on effective programming methods and resident skill development.

Qualifications:

  • Bachelor's degree and three years of experience with individuals with developmental disabilities.
  • Strong communication skills, with the ability to write reports and present information clearly.
  • Valid driver's license and insurance.
  • CPR and First Aid certification.
  • Must pass a background check and drug screenings.
