Job Title: Physical Therapist - Home Health

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality care to our clients in the Seabrook, Hampton, and Exeter areas of New Hampshire.

About the Role:
  • As a Physical Therapist, you will be responsible for assessing and treating clients in their homes, providing individualized care plans to promote independence and improve quality of life.
  • You will work closely with our multidisciplinary team to develop and implement care plans, and provide education and training to clients and their families.
  • Our Physical Therapists are passionate about delivering exceptional care and making a positive impact on the lives of our clients.
Responsibilities:
  • Perform diagnostic tests and measurements to assess client needs and develop individualized care plans.
  • Develop and implement manual therapeutic exercises, gait training, balance, and other interventions to promote client independence and mobility.
  • Continually assess and revise care plans to ensure optimal outcomes and client satisfaction.
  • Provide education and training to clients and their families on rehabilitative care and activities necessary to promote health and safety.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Qualifications:
  • A current New Hampshire Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
