Job Title: Physical Therapist

We are seeking a highly skilled and compassionate Physical Therapist to join our team at Elderwood at Wheatfield. As a Physical Therapist, you will play a vital role in ensuring the health and well-being of our residents by providing comprehensive and quality therapy services.

Responsibilities:
  • Initiate skilled physical therapy treatments in accordance with physician's orders and maintain required contact with physicians.
  • Maintain written evaluations, progress notes, and discharge summaries for individuals in physical therapy programs.
  • Confer with other healthcare staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.
  • Provide input for meetings and attend during absences of the Director Physical Therapy.
  • Maintain statistical records of individuals' treatments.
  • Assist with in-service training programs upon request.
  • Record and update individual's plan of care in the Inter-disciplinary Kardex.
  • Submit requisition for supplies needed in the treatment to Director Physical Therapy.
  • Assume all duties of the Director Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for all necessary documentation.
Qualifications:
  • Graduation from an approved school of physical therapy required.
  • State license to practice physical therapy required.
  • Two years' experience in physical therapy preferred.
  • Ability to direct others necessary.
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
