Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients to identify their healthcare needs and develop individualized care plans.
- Care Coordination: Collaborate with healthcare providers, patients, and families to ensure timely and effective care transitions.
- Communication: Maintain open and transparent communication with patients, families, and healthcare teams to ensure informed decision-making and care coordination.
- Documentation: Accurately document patient information, care plans, and outcomes in accordance with regulatory requirements and company policies.
- Community Resources: Identify and connect patients with community resources to address social determinants of health and promote health equity.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care and outcomes.
- Education: Current CA RN or LVN license, current CPR certification, and valid CA Driver's license.
- Experience: Utilization or HMO experience preferred, acute care or case management experience also preferred.
