Service Coordinator Day Habilitation Specialist

We are seeking a highly motivated and compassionate Service Coordinator to join our Day Habilitation team at Seven Hills Foundation. As a Service Coordinator, you will play a vital role in supporting individuals with disabilities to achieve their goals and live fulfilling lives.
Key Responsibilities- Act as Case Manager for individuals, including case record maintenance and completing ISP requirements.
- Work directly with program participants and their significant team members to develop and follow up on Individual Support Plans (ISP).
- Collaborate with individuals, families, and community agencies to ensure seamless support and services.
- Provide assistance with personal care needs, including dining, toileting, and transfers.
- Develop and implement behavior management interventions in conjunction with the clinical team.
- Participate in program and agency committees as assigned.
- Design and implement instructional units to meet individual needs and interests.
- Assist the Program Manager in screening prospective participants.
- Foster a growth-oriented environment by teaching and offering opportunities for choice, initiation, and risk-taking.
- Evaluate individuals' needs in areas such as activities of daily living, meal preparation, and occupational skills.
- High school diploma or GED.
- Experience with case management.
- Valid driver's license required.
