Job Summary

We are seeking a highly motivated and compassionate Service Coordinator to join our Day Habilitation team at Seven Hills Foundation. As a Service Coordinator, you will play a vital role in supporting individuals with disabilities to achieve their goals and live fulfilling lives.

Key Responsibilities
  • Act as Case Manager for individuals, including case record maintenance and completing ISP requirements.
  • Work directly with program participants and their significant team members to develop and follow up on Individual Support Plans (ISP).
  • Collaborate with individuals, families, and community agencies to ensure seamless support and services.
  • Provide assistance with personal care needs, including dining, toileting, and transfers.
  • Develop and implement behavior management interventions in conjunction with the clinical team.
  • Participate in program and agency committees as assigned.
  • Design and implement instructional units to meet individual needs and interests.
  • Assist the Program Manager in screening prospective participants.
  • Foster a growth-oriented environment by teaching and offering opportunities for choice, initiation, and risk-taking.
  • Evaluate individuals' needs in areas such as activities of daily living, meal preparation, and occupational skills.
Qualifications
  • High school diploma or GED.
  • Experience with case management.
  • Valid driver's license required.
