We are seeking a skilled Hospice Registered Nurse Case Manager to join our team at Signature Healthcare at Home. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and families in their homes and communities.
Responsibilities:- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide professional nursing care, including administering medications and treatments as prescribed by physicians.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Act as a case manager, coordinating patient care for assigned caseloads and ensuring timely and effective interventions.
- Supervise hospice aides and provide guidance and support as needed.
- Current licensure to practice professional nursing in the state.
- Graduate of a National League for Nursing accredited school of nursing.
- Minimum of two years' experience in public health, home care, or hospice nursing, with at least one year in a leadership or supervisory role.
- Excellent observation, problem-solving, verbal, and written communication skills.
- Ability to work independently with minimal supervision and prioritize workload effectively.
