Mass General Brigham is seeking a skilled Registered Nurse to join our team as a Home Health Navigator. This role is responsible for identifying eligible patients and conducting clinical and social eligibility and safety screening in the Emergency Department.
Key Responsibilities:- Attend daily huddles with the Home Hospital team to gather clinical information and identify patients based on catchment area and plan of care.
- Interact with the Emergency Department interdisciplinary team and ED Care Facilitators to assist patient identification and transfer to MGB Home Hospital.
- Collaborate closely with the MGB Home Hospital Admitting Provider to notify patients who meet eligibility criteria and obtain consent for transfer.
- Communicate and collaborate with Administrative Coordinator and Administrative Manager regarding patient transfers and manage patient flow and placement to support Home Hospital transfers and census goals.
- Utilize analytical and critical thinking in decision making and judgment that is aligned with organizational objectives.
- Current RN licensure in the state of Massachusetts.
- Minimum 3 years emergency department, home hospital, or inpatient experience.
- Varied experience preferred.
