Job Summary

This role focuses on providing high-quality nursing care services in accordance with physician orders. As a Nurse Navigator, you will play a crucial role in guiding patients through the healthcare system, overcoming obstacles, and ensuring timely access to care.

Key Responsibilities
  • Assess and facilitate coordination of multidisciplinary care across the care continuum.
  • Provide communication, advocacy, and education in a culturally competent manner.
  • Triage new patient referrals to ensure timely access to care.
  • Conduct post-discharge follow-up phone calls and serve as a liaison and patient advocate in navigating the healthcare system.
  • Provide patient education and support utilizing knowledge of adult learning competencies and education resources to support complete, connected, and comprehensive care.
  • Collect and report data from patients on barriers, hospital experience, healthcare, medications, prior treatments, etc.
  • Collaborate with the multidisciplinary care team and expedite the sequence of patient care.
  • Serve as a liaison between patients and providers/clinicians to address patient needs.
  • Assess patient needs and ensure such needs are integrated into patient care plans.
  • Encourage and empower patients to actively participate in decision-making about their treatment and care.
