Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team in Menominee. As a Visit Nurse, you will provide direct nursing care to patients in their homes, working under the supervision of a Registered Nurse and/or designated supervisor.

Key Responsibilities
  • Provide high-quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the healthcare team.
  • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
  • Actively participate in the agency's care coordination process, including timely reporting of patients' needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other healthcare professionals.
  • Ensure confidentiality of patient health information per agency policy.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
